I would pull the valve covers off and check that injector like the guys said. at 40PSI is should be clear what the problem is. I'm guessing with no coolant in there that it is an injector not seated.
I don't, it was puking coolant at 110K miles. I figured lifted a head. didn't expect it to look like 200K plus miles inside. didn't notice a lot of blow by on it
since my nice starretts all take the same battery and they are dead, my el cheapo caliper says that #8 has about 5 thou taper and 3 thou out of round. #6 has 2-3 thou taper and just under 2 out of round. tells me time for some lower work.

change the fuel filter. tune the programer down. If you want power get EFI, it is safer for the truck. If it pops the code with the programer turned down and a new filter shim the FPRV
60-100 overs depending on what HP you want. smaller is better for DD, but I wish I had at least 100 overs in my DD. Seems like if you go with a good set 100 overs still DD ok
