sounds like just some bad cups
was pressurizing the coolant the only problem

started losing power on the way home from suncoast last year and as the power loss continually got worse the white smoke started pouring out with that heavy raw diesel smell...my guess is more than the one injector I knew was bad let go...
still hard to explain the balance rates all being 7.0 though... I know these electronics are made to compensate for issues going on but all the other seven injectors balance rates went to 7.0
note:coolant pressurization and 1 bad injector(7.0 rate and some hazing) was known before suncoast
