I worry about something like this every night
I deal with other peoples accidents every day. Here's a couple pics of recent jobs

I do this as well hell I stop again bout 2ft off and check again then I make sure it latches correct before going under and shake again
I'll share a story relating to a company that my uncles runs. They are a transportation company and he is in charge of their other business affairs which mostly are transporting hay to china. The drivers show up with their trucks as huge tractors load large bails if hay, equivalent to the size of a semi trailer rear door (10'x10' or however large they can make to fit.)
Well this driver showed up and opened his truck, and had to go remove a strap in the trailer. The drivers had strict rules to stay in the cab of the truck. While he was in the trailer, the operator of the tractor loaded the first bale of hay (the tractor operator cannot see over the hay) There is maybe 2" of gap on each side between the hay and the trailer walls. He continued to load the cubes of hay and at the end they compact it all in... The tractor operator then went to look for the driver and realized he was no where to be found, and realized his mistake... The driver either died of suffocation or was crushed to death. Sad story for both the driver and the tractor operator. Terrible accident. Now the drivers have to stand on the side of the trailer within eyesight of the tractor.

Local Cat dealer had a D9T on a lowboy. The tractor was not loaded correctly and the driver played chicken with the overpass and the ROPS on the dozer... Not sure who really lost but both were in bad shape as it tried to rip the D9 off the low boy.

after watching me drive AROUND the conrete light post base the Hogan driver drove the truck over and manga to get the trailer stuck on it.
i backed under this one day.. i didnt feel the BUMP in the normal spot..so i stopped and looked 2" clearance..
another trailer.. what happens when you try to align the trailer with out taking the weight off the landing gears..
fell alseep IDK it wasnt there when i came in that night but there when i left in the AM guard rail 1 truck -1
i was told the yard mule was repsonible for this
mechanically sound trailer?

Same here. I also use some high lift jack stands under the front and back to make sure everything is stable. My lift manufacturer actually recommends it when removing heavy objects, but it makes it so much more stable, I just do it all the time. A few things luckily it was a late model and easily replaced for the most part. Imagine a boat load of accessories (upgrades) on a early model worth way more then book value! As ultimately its a insurance company that is buying the new truck!We know how they will be. A dealer is only going to do so much.

I know of lots of trucks falling off lifts. One of my buddies Dmaxs fell off. He had Step bars just like mine and they held his truck up. The step bent up just enough to put a small dent in the door and that was it.
I can't take pictures at work since we work in a lot of chemical plants. I can tell you a 5000gal tanker of acetone IIRC burning is an amazing site. Unfortunately it was ignited by the guy on top of it when his cell rang.
