Looks like foreign object damage. My guess is something left over from the old turbo that got missed. Did you clean out the tubing all the way down to the intercooler?

Intercooler was pulled and washed several times with air blasts between washes in both directions....

I'll have to look into that. The filter was pretty dirty as I'm not the one driving it all the time now.

I thought the same thing. Would a shop towel have made any damage similar to this. The damage seems like it was something a little more dense.

My opinion, it doesn't look like a rag or shop towel. Definitely something hard. Maybe a piece of aluminum or possibly even hard plastic

Debris in the intercooler piping wouldn't cause this damage. However if you didn't replace the air filter, only cleaned it best as you can, may have been some metal hiding in the pleats. Your previous failure was extensive enough to send plenty of material to the air filter

This is what I think it was. IMO the filter should have been replaced to eliminate any concern/potential for missed debris. I don't think we will ever be able to 100% know what this is from but it definitely looks like something hard made it in there.
