Not to get in the middle of the expert talk but have you done any failure analysis or just kept throwing injectors at it? Sent fuel out to be tested for contaminants, water, etc? Checked tank, fuel lines, made sure you're not blowing holes in filter media? Going down to the smallest micron filter might seem like the best idea unless you're running lots of pump and shoving filter media into your injectors.
I'm a nobody in the injection world. Just never been much for throwing parts at the situation without ever figuring out the base cause. Things don't just fail for no reason, there's always a cause. Even if it seems random from truck to truck. Just my $.02, not worth much. Hope you get it worked out.
