I had close to the same issue on my old lly, ended up having an issue in the factory fuel filter housing. It had no lift pump at the time. We installed clear hose and were able to see the air. Also checked the restriction and had about 11" of vacuum at an idle and rail pressure was dropping out bad on it. I deleted the factory fuel filter and installed an air dog pump.
