What about the oil cooler? Just a thought, idk...

I guess you missed the part where coolant was not only getting into the oil, but also the cylinders. I took the heads to the machine shop and upon handing them over I noticed where some pitting had occurred between water jacket and one of the valves. This explains the coolant getting into the cylinder. We'll see what else they find as they tear it down.
