Check the fuel filter if it has not been replaced lately, could be plugged. if recently replaced, could be o-ring leaking.

Pump primer on filter head assembly and see if it ever gets hard to push anymore. If it doesn't would indicate leaking and drawing in air. Also sometimes just pumping it up will show a leak at the filter head. I've seen it slot around fuel heater

I replaced the fuel filter about 1500 miles or so ago and the problem is just now coming up but I'm thinking about getting rid of stock filter assembly all together beings I have a lift pump and prior to me owning the truck the stock assembly was converted to the caterpillar filtration any opinions?

I've got a lift pump and run 0749 cat filters.

Shouldn't start hard with a lift pump unless an injector or injectors are bad.

I would check the lift pump pressure at the filter head to see if it's putting out and if so, I'd check rail pressure with the truck off and then see what cranking pressure is.
