Someone with Adobe PDF help me out....

I have this PDF and all I can do is view it, I need a couple thing filled in so I can e-mail it but it wont let me. If someone wants to take a stab at it and fill in a few blanks I'd greatly appreciate it.

What I need done:



Checkmark on number 8

Fill in bottom as "Mike Meyer" where it says importer
fill in at bottom next to my name - 8300 Desert Quail Dr. Las Vegas, NV. 89128 for my address, and thats it.

Unfortunately - it's not a fillable form. If you still can't get it, I can print it out, type in answers, scan it back in and email it to you.

I opened in photoshop CS, filled in the stuff, safed it back as pdf ... here ...

Adobe professional will let you insert text boxes or the typing tool works even better. Problem with that is if you open the doc with adobe reader you can't modify the boxes...
