Help: Power steering pump

My dad has an 04 Lb7 and while driving the pulley come off of the power steering pump. It looks like it is pressed on. Any idea why it would have come off? Do I need a new pump in order to fix this. Thanks James

hows the shaft look? im going to say new pump,but not sure I never heard of that happening before

Did the belt get destroyed? any other damage?
does the shaft still spin?

Belt is fine,to my knowledge everything else is working fine.
