I just wanted to start a short thread here since when I was looking on various forums I could never find concrete update on this. Maybe it could help someone out at some point. I was fixing to dig into check the reluctor wheel and start changing sensors.

My 05 LLY started throwing an occasional P0016 and P0698 code while on the road a while back. At first I didn't notice much for symptoms but they started to progress.

Background on the truck is an 05 LLY with 270k miles, stock motor, just BPU with a built trans.

Symptoms I experienced:

Lurching/loping ahead when in drive with <15% accelerator pedal position and speeds under 10 mph
Loping at highway speed (similar to stabbing the pedal and letting off again)
Occasionally I could have no input from the accelerator pedal EXCEPT when it was to the floor and then it would max out at 30% position

The truck always started, and if you let it sit for 10-15 min the accelerator pedal would come back. If you cleared the codes, the symptoms would go away but would comeback soon after depending on how far I was driving. But I never once got a code for the accelerator voltage. The correlation does make sense as the Cam Position Sensor and Accelerator Position Sensor are on the same circuit.

After doing some research I found that some have had success with cleaning the bail connectors on the driver's valve cover. I took them apart and inspected all the pins and everything looked tight and clean. Regardless I ended up spraying some electrical contact cleaner in it as well.

Upon re-assembly it seemed to have alleviated the problem all together. Its been 2 weeks now with no issues. It was throwing the code multiple times a day prior to. Just goes to show you the sometimes its the simple things, even if its not obvious.
