EFI tables, which ones?

Been browsing through EFI, getting used to the landscape, and ran across some questions on tables with similar parameters:

1) On the base injection timing tables, there are five separate ones, A-E. The notes say that Table B is the most common table used most of the time during normal running. Under what conditions are tables A,C,D and E referenced?

2) References are made under several calibrations to High, Medium and Low altitudes. What are the differences? Where does Low altitude end and Medium begin?

There is a "TIMING TABLE" PID that says which table you are running on. Normally these tables are altitude related, but I seldom if ever see it in any table other than B.

The altitude 'breakpoints' are programmable. See B0202. Unlike earlier O/S's, this method can "feather" from one altitude to another.

If you want fixed altitude tuning, just make SEA column all 1.00, and the HIGH / MEDIUM columns all zero.
