As far as I can tell the outer vains fatigued and broke then it was over from there. It really didn't have a tough life, about 20k behind my LB7 back in the day at around 500rwhp. Then 35-40k behind a twinned LML with smallish tow tunes hauling a 5th wheel and some toys. Converter clutch looked new other than the metal embedded in it.
