checked my balance rates today most was +4 to +5 and two were +6 , is this good or bad?

Tested with engine up to operating temp? In drive or park? Stock tune?

engine warm, in park , ppe level 3 tune

Put the stock tune in and recheck it in park at temp. Also take note of what teh fuel rate is at idle. Not sure on the PPE, but I know some scanners will read the balance rates incorrectly on the LLY. I know if I use my V2 to check them as a stand alone scanner it will read all over the place, but if I hook up the laptop to it and use it in pass through they read normal.

Try cleaning your injectors to bring balance rates down.

what should the fuel rate be?

Roughly 8-9MM3 of fuel at an idle. If it is say 5, then you need to add 3 to all your balance rates to get the true number. But if it is 12, then you need to subtract 3 to get the true number.
